Here are instructions for requesting a recommendation letter from me: in your request email, please send along the following information:

• If you've taken courses with me, let me know which courses you've taken with me and when.
• Write down a few paragraphs answering the following questions:
  • What is your story?
  • What are you passionate about (besides math and, especially, besides whatever job it is that you are applying for)?
  • What have you learned about yourself in college?
  • What hidden talents do you have? (You may have excellent organizational skills, or an amazing memory, etc.)
  • What do you aspire to do with your life?
  • Was there something about any one of the classes you took with me that I should know about? (For example, maybe you fell in love with epsilons and deltas while taking my real analysis course, or maybe you recovered sharply from a bad test result, and you wish to remind me of that.)
